YOU WILL PLEASE TAKE NOTICE that on or about October 19, 1994, the project,
CMS95A, entitled "Pavement Rehablitation and Overlay Project F.Y. 1994-95, Phase III",
consisting of the furnishing and installation of Type III slurry on Highway 101 in the City of
Encinitas, in which CALIFORNIA PAVEMENT MAINTENANCE COMPANY, INC. was the
contractor and the Reliance Insurance Company of California was the surety, was completed.
