NOTICE OF COMPLETION
1. Pursuant to the Contract fcr the Ulen Park Site Improvemenk - s
(Americans With Disabilities Act Compliance) Project, awarded to T
& M Construction, did o:i December 16, 1993 enter into an ag.reement
with the City of Encinitas to construct rertain improvements
indicated in contract documents, said improvements consisting of
grading and installing concrete walkways; renovating existing
public restroom; removing and constructing a new restroom roof;
constructing a disabled parking space; renovating doors and adding
a one-stall restroom to the Scouthouse building.
2. Said agreement, including p?ans and specifications fo.r said
improvements, is on file in the Office of the City Clerk of the
City of Encinitas.
3. The name of the surety or the Contractor's boncis guaranteeing
constructJon af said improvements and the payment of labor and
material therefore is Amtvest Surety Insurance Company, Los Angeles,
California with policies issued on December 20, 1.993.
4. Said improvements WPre comp)_eted anC. accepted by the City
Council of the City of Encinitas on the 25tY, day of May 1994.
5. Performance Bond #C93003920, in the amount of $74,880.00 is
hereby released on May 31, 1995.
6. Labor and Ma*_erial Paymerit Bond #003003920 the amount of
$74,880.00 is hereby released on July 15, 1994.
7. Neither acceptanr_e ot said improvements by the City Couricil of
the City of Encinitas no-r tYzis notice constitures a waiver of any
defects in the work of the construcLion or installation of said
improvements.
